University Health Shreveport (UHS) and the City of Shreveport-Public Assembly and Recreation (SPAR) have established a strategic collaboration aimed at strengthening clinical-community linkages and improving the quality of life for Shreveport residents suffering from arthritis and other chronic conditions. Thanks to a twenty thousand dollar ($20,000) grant from the National Recreation and Park Association, UHS and SPAR will develop and test an innovative strategy to promote healthy living and encourage participation in proven intervention programs offered through parks and recreation.
University Health Shreveport is excited to have the opportunity to refer their patients to such a much needed program. The community partnership compliments our medical care as we are always interested in programs that help our patients being healthier. More than ever before health systems are establishing results-driven partnerships with community-based organizations in efforts to enhance care coordination services in communities, improve patient access to preventive and chronic care services and the overall reach of community-based programs that help patients lead healthy lifestyles.
